Athletic Physicals – May 31

IMPORTANT DATES FOR ALL STUDENT ATHLETES

FREE ATHLETIC PHYSICALS

Required every year for all student athletes entering 5^th-12^th Grade

TUESDAY, May 31^st at 6pm in the Healthplex

OPTIONAL BASELINE CONCUSSION SCREENINGS

Tuesday, June 7^th at 2:00pm in the Middle School Computer Lab (Building B) – Charge $30

All students must complete the Parkview athletic packet ONLINE prior to arriving at physicals on May 31^st.  LHSAA forms must be printed, filled out by hand, and brought to physicals.

Athletic packet for Parkview (Completed online)

• Demographic information for Student-Athlete
• Hold-Back Repeat Student Tracking Form
• Participation in Extracurricular events – PBS Assumption of Responsibility form
• Guarantee of Medical/Hospitalization Coverage (must upload copy of insurance card or bring copy to physicals)
• Acknowledgement of Student Athletic Handbook and Consent Form. Please make sure to read the updated academic requirements in the PBS Athletic Handbook before simply signing off.
• Birth Certificate (you may upload this when filling out packet or bring a paper copy to the physicals.)

LHSAA FORMS: All students should DOWNLOAD the LHSAA paper forms and have all filled out (except the physical) prior to arriving at physicals.

• 7^th/8^th Grade Student Checklist and Eligibility
• LHSAA Medical History and Physical Form (top of form should be filled out prior to physicals and bottom of form (physical) will be filled out by physicians on May 31^st
