FREE Athletic Physicals
FREE physicals (required for ALL student athletes 5th-12th grade) will be held in the Healthplex next Tuesday, May 30th beginning at 6pm.
Optional Heart Screenings (no charge) will take place that same day beginning at 4:30pm in G Building (Elementary). Click here to fill out the heart screening permission form. Heart Screenings are for NEW students (grades 9-12) that have never been screened before, as well as all 8th grade athletes and spirit group participants.
You must have BOTH the LHSAA and PBS packets filled out BEFORE arriving at Physicals. Please click here to download both packets.
