Parkview Baptist School requires a copy of your child’s birth certificate (not a copy of hospital certificate) upon enrollment. Copies of current immunization records and birth certificate are required when submitting the Application for Admission.

Please Note: If your child attended Parkview Baptist Pre-School, you are still required to provide a copy of your child’s immunization records and birth certificate as these vital records are not transferred.

If you do not have a copy of your child’s birth certificate, please apply to the state immediately. The state will issue you a “certified copy” of the original certificate. Each state archives an original and issues certified copies to parents, legal guardians, or other such qualified individuals. Please submit only a plain copy of your certified copy.

According to the State of Louisiana, certified copies for children born in Louisiana may be obtained in Baton Rouge at the corner of Florida Boulevard and North 12th Street. Persons requesting a certified copy must be an immediate family member and must show some form of Louisiana Identification (driver’s license, etc.) Only cash, money orders, cashier’s checks or personal checks are accepted. (No credit cards)
